BCUEA Delta Dental Plan Information

BCUEA Delta Dental Plan Open Enrollment is September 1-30, 2010 only.

INDIVIDUAL (Employee Only)  $292.40 annually - $14.62/pay for 20 pays

TWO PERSON (Employee & Spouse or Employee and One Dependent)  $584.80 annually - $29.24/pay for 20 pays

FAMILY (Employee & Family)  $695.40 annually - $34.77/pay for 20 pays

First payroll deduction will be Sept. 24, 2010.

Open enrollment open to: non-instructional BCUEA members employed by Bethlehem Central School District who work a minimum of half time or more and have been employed continuously for a minimum of one (1) year prior to October 1st.

Changes to your enrollment:

• Change of marital status, change of address/phone, etc.
• If you have a child over 19 years of age and not attending college, they must come off your policy. If they are attending college we need proof in the form of a letter from the college stating they are enrolled and attending full time.

Please see your Building Rep or Building Secretary for Enrollment and Payroll Deduction Authorization forms.
After completing all the information, return ALL forms to Marge Deveneau at the High School. DO NOT send these forms to the Business Office.
