Available Plans:

INDIVIDUAL (Employee Only)

$300 annually  -  $15 /pay for 20 pays

TWO PERSON (Employee & Spouse or Employee & One Dependent)

$600 annually  -  $30 /pay for 20 pays

FAMILY (Employee & more than 2 family members)

$760 annually  -  $38 /pay for 20 pays

Please Note: Due to administrative fee increases by Delta we have had to raise the rates of our plans slightly. To counteract some of the raises and in attempt to keep the rate increases to a minimum, we were also able to negotiate a couple of small increases in our benefits. Beginning Oct. 1, 2011 the Basic Restorative (fillings) will increase from 80% coverage to 100% coverage. Major Restorative (crowns) will increase from 60% coverage to 70% coverage and Frequency Limitations for prophylaxes/exams (cleanings) will change from twice in any 12 month period to four times in any 12 month period.

Note:  First payroll deduction will begin on September 23, 2011

Enrollment is open to non-instructional BCUEA members employed by Bethlehem Central School District who work a minimum

of half time or more and have been employed continuously for a minimum of one (1) year prior to October 1st. See your

building rep or building secretary for Enrollment and Payroll Deduction Authorization forms starting  in September.
